General Instructions

1. All documents must be in English or include an official English translation

2. Provide each of the following as part of your application package:

  • Application
    The Application Form (PDF) may be sent to us electronically by using the submit button at the bottom of the form, or you may print it and send it by e-mail, fax or postal mail as specified below.

  • Two References
    These should be from people who are familiar with your educational and professional qualifications. Standard letters of reference are acceptable, or you may use our Reference Form (PDF). Your referees can send us the form electronically by using the submit button at the bottom of the form, or they can send it by e-mail, fax or postal mail as specified below.

  • Résumé or Curriculum Vitae
    Your resume should include academic history, employment history, relevant experiences and publications list. Submit your résumé as specified below.

  • Official Transcript
    This should be an official transcript of your most recent degree or the degree that qualifies you for this program. Copies of official transcripts are accepted. Sumbit your transcript (or have your school submit it) as specified below.
