End of Appointment

Research appointment end dates are specified in the appointment letter (and any subsequent extension letters) that a participant receives from ORISE. Appointment end dates should be strictly observed and may not be changed without the appropriate communication to ORISE.

As a participant’s scheduled end date approaches, ORISE will send a letter confirming the end date and outlining the procedures necessary to complete the appointment. The participant will need to submit a brief report (one to two pages in length) describing the research and educational experiences during the appointment and evaluating the assistance provided by the CDC and ORISE. Any outstanding travel reimbursement forms must also be submitted.  These items must be received by ORISE before the final stipend can be issued.

Participants who are enrolled in the ORISE insurance plan will receive a COBRA election form detailing the options for continuing their coverage.

Leaving an Appointment Early

Should a participant need to leave the program before the specified end date, the participant should directly inform their ORISE point of contact, their mentor and their program coordinator.  Two weeks’ notice is requested. Providing insufficient notice, or failure to communicate directly with ORISE, could result in unnecessary complications with final stipend payments.

Extending an Appointment

Continuing participation after the end date specified in the most recent appointment/extension letter, even by a single day, is not permitted as it requires an appointment extension. Please see Renewal of Appointment for more information on this process.
