TA-16 Stepladder Accident Investigation (Video)
The TA-16 Stepladder Accident Investigation video was developed at Los Alamos National Laboratory (LANL) by the Health, Safety, and Radiation Protection Division and the Strategic Research Directorate.

This video was produced as part of a LANL investigation of a stepladder accident that occurred in April 2003. LANL chartered a team to investigate the accident and prepare an investigation report in accordance with DOE Order 225.1A, Accident Investigations. The accident occurred when a subcontractor employee was standing on an 8-foot stepladder using a cutting torch to remove an overhead 6-inch-diameter carbon-steel pipe. As the worker let the pipe freefall to the floor, the pipe struck the stepladder, causing the worker to fall and injure his right leg.

The 7-minute video can be used in a variety of ways-as a lesson-learned tool, refresher training in safety meetings, and other formats.
