Principal Investigators should complete this form to provide all information necessary for a rapid and thorough review of their proposed research by the Central Be IRB.

Continuing Review - Application to Involve Human Subjects in Research (Word)
This form must be completed in order to continue research that was approved by the Central Be IRB within 12 months of the previous approval.

Modification, Change, New Finding, or Minor Adverse Event (Word)
This form should be completed if a change to the research study or protocol, or a new finding, would change or increase the risk to the human subjects participating.
